Confidentiality
On your first visit you will be asked to complete an intake form which will familiarize
your therapist with your health history and medications you may be taking. It is
imperative that you complete the intake form completely and accurately since certain
medical conditions make massage or bodywork inadvisable.
The massage and bodywork profession has a strict code of ethics which puts the
respect of the client first and foremost. All client notes, conversations, and information
are held in confidence. In the event that information regarding a client needs to be
shared with a third party, the client will be asked to sign a “consent to release”
authorization form.
Etiquette
Please silence your cell phone during treatment for maximum relaxation
Insurance Requirements
For liability purposes, only the client receiving treatment is allowed on the premises.
Please make arrangements for child care prior to your appointment.
Cancellations, Tardiness, and Missed Appointments
Please notify us 24 hours in advance to change or cancel your appointment. Solace
Massage Therapy reserves the right to charge for no shows and appointments cancelled
with less than 24 hours advanced notice.
It may be necessary to forfeit the missed portion of a scheduled service for clients
arriving late, however you will be responsible for full payment of your scheduled
appointment.
Improper Actions
Any illicit language, sexually suggestive remarks, actions, or advances will result in the
immediate termination of the session.
